How to Request an Online Consultation™

Online Consultation™ is not intended as a substitute for a physician examination or for emergencies. If you have a true, life-threatening emergency, dial 911. However, if you have an issue that you would like to discuss with the doctor but you are unable to bring your child in for evaluation, you may request an online physician consultation. The cost for this service is $25 and it is non-refundable. If you have further questions regarding Online Consultation™ please see the FAQ section for more information. If you are ready to request an Online Consultation™ then please follow the instructions below:

1. If you have not already done so, click HERE register for an account with Medem®. If you already have a Medem® account, skip to step 3.
2. Once you have registered, Dr. Alvis approves each individual account that is requested (to ensure only his patients receive access). You will receive a confirmation e-mail notifying you when your account is approved (usually within 24 hours).
3. Log on to our Medem® web site by clicking HERE (this will open a new window).
4. Click "Message Inbox" on the top toolbar then "Online Consultation™ Request" on the left toolbar.
5. Fill out all of the fields as completely as possible. A physician will respond to you as soon as possible.
